Insurance
Dental Insurance and Co-Pays:
It is a courtesy to our patients that we bill your dental insurance, however we do require current & accurate dental insurance information. The balance is your responsibility whether your insurance pays or not. In addition to your estimated co-pay (paid at the time of treatment), if your insurance does not cover your treatment, that balance is your responsibility.
Your dental insurance is a product designed by your employer. Your benefits are dictated by the choices made within the cost of the coverage. WE will do everything possible to maximize your insurance benefits; however, please do not let the lack of benefits dictate your desired or necessary treatment. Take charge of your dental health! We will be happy to help answer any questions or concerns you have regarding your dental treatment and insurance coverage.
Payments
We are committed to providing the best treatment for our patients. We do our best to provide our patients with a clear explanation of all fees associated with your treatment. The estimates are based on the most current information available. Insurance companies will not guarantee payment to the dental office, therefore, what we provide you with is just an estimate. If the insurance plan does not pay or if you are not covered for any reason, you are responsible for the balance.
We accept all major credit cards: Visa, Mastercard, Discover and American Express. We also offer outside financing through Care Credit. You can apply online at www.carecredit.com or call (800)365-8295. They offer No Interest Payment Plans for 6 or 12 months.
Our Office Policy
Missed Appointments: Your scheduled appointment is a time reserved specifically for your. As a courtesy to our patients we do call or email to remind patients of their appointments. Unless canceled, at least 48 hours in advance, our policy is to charge a minimum of $50.00 for missed appointments, or a fee equivalent to the time reserved for you at each missed appointment. Please help us avoid these charges to you by keeping your scheduled appointments.
Statement Charge: Once again, payment is due at the time of treatment. Any balances remaining after the insurance has completed their payment will be billed to the patient by statement. Any unpaid balance that requires additional statements may be subject to a $10.00 statement fee for each additional statement.
